Treatment of infections with multidrug-resistant tuberculosis.
To review the various treatment options for multidrug-resistant (MDR) tuberculosis (TB) and to determine the most effective regimen in this situation. A literature review and analysis of available treatment regimens was performed. All cases were treated in the standard six-month intensive phase consisting of isoniazid, rifampin, pyrazinamide and ethambutol. The four-drug regimens consist of ethambutol, streptomycin, rifampin and cycloserine; and the two-drug regimen consists of clofazimine and cycloserine. The regimens with clofazimine are not approved in the US. The regimen with cycloserine has not been evaluated in humans. The four-drug regimen has not been compared to any standard regimen, but it appears to be more effective than the two-drug regimen. Mortality rates range from 18% to 46%.
